The visual system of vertebrates has greatly contributed to our understanding of how different molecular mechanisms shape adaptive phenotypic diversity. Extensive work on African cichlid fishes has shown how variation in opsin gene expression mediates diversification as well as convergent evolution in colour vision. This trait has received less attention in Neotropical cichlids, the sister lineage to African cichlids, but the work done so far led to the conclusion that colour vision is **** less variable in Neotropical species. Here, we survey patterns of opsin gene expression in 35 representative species of Neotropical cichlids, revealing **** more variation than previously known. This diversity can be attributed to two main mechanisms (i) differential expression of the blue-sensitive sws2a, the green-sensitive rh2a, and the red-sensitive lws opsin genes, and (ii) simultaneous expression of up to five opsin genes, instead of only three as commonly found, in a striking dorsoventral pattern across the retina. This intraretinal variation in opsin genes expression results in steep gradients in visual sensitivity that may represent a convergent adaptation to clear waters with broad light environments. The 891 athletes (mean 14.8 years, 35% girls) practiced 45 different sports on three different levels, representing all types of static and dynamic composition of the Classification of Sports by Mitchell. There were 75.4% of normal ECG findings, among them most commonly early repolarization, sinus bradycardia, and left ventricular hypertrophy; 4.3% had a borderline finding; 2.1% were abnormal and required further investigations, without SCD-related diagnosis. While the normal ECG findings were related to sex, age, and endurance sports, no such observation was found for borderline or abnormal criteria. Our results in an entirely pediatric population of athletes demonstrate that sex, age, and type of sports correlate with normal ECG findings. Abnormal ECG findings in pediatric athletes are rare. The International Criteria for ECG Interpretation in Athletes are appropriate for this age group.Excimer light is an effective therapy for psoriasis resulting in faster clearance and a longer remission. We studied its additional effect in patients with psoriasis under other therapies. Subjects were composed of 261 patients with psoriasis, all of which were treated with topical application of steroid and vitamin D mixed ointment. Other therapies were composed of apremilast and biologics. PASI 100 rate were evaluated until 12 months after the start of its therapy. PASI 100 rate was elevated from 13 patients (5.0%) to 34 patients (13.0%, P less then 0.01). Twenty-one of 63 patients (33.3%, P less then 0.01) not reaching PASI 100 reached it after excimer light therapy. The addition of excimer light therapy in each treatment showed no significant difference compared with that before addition. Irradiation amount in patients with guselkumab (822 ± 259 mJ) was significantly smaller that with excimer only (1370 + 731 mJ) and that with IL-17 (1747 ± 749 mJ). In addition, irradiation amount in patients reached PASI-100 in Guselkumab (717 ± 75 mJ) which was significantly smaller that in the excimer only group (1530 ± 450 mJ). Additional effect of excimer light therapy in psoriasis, especially in patients with guselkumab, was speculated.

A 46-year-old male patient with an implantable cardioverter-defibrillator was admitted for recurrent ventricular tachycardia requiring repeated shock refractory to antiarrhythmic drugs. Although the patient was previously diagnosed with hypertrophic cardiomyopathy, multimodality imaging studies showed large left ventricular intramural AVMs, potentially representing arrhythmogenic substrates. Life-threatening ventricular arrhythmia storm, which could not be controlled by radiofrequency catheter ablation and therapeutic hypothermia. However, cardiac sympathetic denervation surgery successfully controlled the ventricular arrhythmia storm.
