Mechanisms regulated by IL23 included induction of PDK1-dependent bacterial uptake, and up-regulation of reactive oxygen species through reduced nicotinamide adenine dinucleotide phosphate oxidase members, NOS2, and autophagy through ATG5 and ATG16L1. Enrichment rearing enhanced direct interactions with the stranger mouse in all **** in the three-chamber social interaction test however, NL3R451C **** did not show impairment in social interaction in the three-chamber test, in contrast with previous reports. However, the precise %fT>****target associated with improved clinical outcomes is unknown.The aim of this study was to investigate the correlation between survival of patients with Pseudomonas aeruginosa bacteraemia and the threshold of Piperacillin + Tazobactam %fT>MIC. This retrospective study included all adult patients hospitalized over an 82 month period with Pseudomonas aeruginosa bacteraemia, and treated with Piperacillin + Tazobactam . Patients with a polymicrobial infection or those who died within 72 hours of culture, were excluded. The %fT>****of Piperacillin + Tazobactam associated with in-hospital survival was derived using Classification and Regression Tree analysis. After screening 270 patients, 78 were eligible for inclusion in the study; 18% died during hospitalization. Classification and Regression Tree analysis identified %fT>MIC >60.68% as associated with improved survival, and this remained statistically significant after controlling for clinical covariates (OR= 7.74, 95% CI 1.32-45.2). In conclusion, the findings recommend dosing of Piperacillin + Tazobactam with the aim of achieving a pharmacodynamic target of at least 60% fT>****in these patients.Therapeutic options for Mycobacterium abscessus infections are extremely limited. New or repurposed drugs are needed. In vivo studies showed that BIBW2992 and U0126 significantly reduced parasite burden, and the parasite obtained from BIBW2992-treated **** displayed impaired structural integrity of the germinal layer. Results Paired comparison of pre and post-HBCR using Wilcoxon signed-rank test revealed a statistically significant difference in the pre and post-HBCR scores for DASI, DASI-Mets, and SF-36 physical functioning (p values 0.05, 0.034, and 0.016, respectively), suggesting an improvement in the patients' physical functioning after participating in the HBCR program. Patients treated before August 2013 underwent the traditional Dresden protocol, and those treated subsequently received the accelerated protocol, i.e., 0.1% riboflavin soaking for 10 min, followed by pulsed UV-A irradiation at 30 mW/cm2 for 8 min, and a total irradiation power of 7.2 J/cm2. The primary outcome was the success rate in halting the disease progression. The postoperative changes in visual acuity, keratometry readings, endothelial cell count and complications following accelerated CXL and conventional CXL were compared. Results This study evaluated 113 eyes over a mean follow-up period of 37.61 ± 16.34 months. It was found that 89.6% and 95.7% of the eyes were successfully treated with the traditional and accelerated protocols, respectively (p = 0.239). The corrected-distance visual acuity improved in both groups, but it was significantly better following the accelerated protocol (p = 0.037). Traditional protocol group had more postoperative complications than the accelerated group (8.9% versus 2%), although not reaching statistically significant difference (p = 0.142). There were no significant differences in the topographic (p = 0.122) and refractive (p = 0.570) outcomes. Conclusion Accelerated CXL and conventional CXL are both safe and effective, and the long-term follow-up showed that the accelerated protocol was similar to the standard one in terms of halting keratoconus and achieving visual outcomes. Methods A cross-sectional, pre-piloted 23-item self-administered questionnaire survey was distributed electronically using the Bristol Online Survey tool through the mailing list of the European Academy of Paediatric Dentistry, the Arabian Academy of Paediatric Dentistry's and the Egyptian Society of Paediatric Dentistry. Results 308 respondents completed the survey. Approximately half practised RET (N = 158; 51.3%), which was most frequently used to manage non-vital traumatized teeth. Majority of RET practitioners worked in university or dental institute practices. Protocol differences with deviation from the current published RET guidelines were observed.

Resultados Fueron incluidos 64 pacientes con una edad media de 62 años. El promedio de internación en días fue de 4,9, con 10,9% de reinternaciones y 4,7% de reintervenciones. El 69% de los pacientes no presento complicaciones, registrándose 5 complicaciones mayores (8%). Los vegetales frescos son susceptibles de ser contaminados con estos patógenos durante su cultivo, cosecha, transporte, procesamiento y manipulación. Por ** que se pretendió determinar la frecuencia de detección de NoV en muestras vegetales de hojas de la Ciudad de Córdoba, y adaptar un método de concentración viral con polietilenglicol para la recuperación de partículas virales de la superficie de vegetales y caracterizar los genogrupos de NoV detectados. Métodos Se tomaron 19 muestras de vegetales de hoja entre junio y diciembre de 2012. Se aplicó una técnica de concentración viral validada previamente en el laboratorio (elución y precipitación con polietilenglicol). A los concentrados de las muestras se les extrajo el ARN viral utilizando Trizol y precipitación con alcohol isopropílico. Hacia el año 2016 no hemos hallado información sobre los recursos de los centros médicos en Argentina y su integración en redes de atención. Objetivo Describir los centros médicos relevados y sus recursos para las áreas médicas de cobertura mencionadas. Métodos Es un estudio descriptivo, de corte trasversal entre 12/2016 y 8/2019. El muestreo fue no probabilístico y por conveniencia. Las variables se reportaron como proporciones y las comparaciones se realizaron mediante el test de chi cuadrado o Fischer. Resultados Ingresaron 232 centros, 66.8% en ciudades capitales y 67% del ámbito público. Las capitales se asociaron con mayor presencia de recursos centros categoría 3 (OR 7.85; IC del 95% 3.66-16.84; p less then 0,000001), angiografía (OR 5.94; IC del 95% 3.24-10.28; p less then 0,000001), tomografía (OR 3.41; IC del 95% 1.51-7.69; p=0,002), trombolíticos (OR 3.24; IC del 95% 1.37-7.76; p=0,005); excepto cirugía de trauma (OR 1.83; IC del 95% 0.75-4.46; p=0,17). Sin embargo, el resultado de la biopsia puede no ser concluyente en más del 40% de los casos. El objetivo de este estudio fue revisar el manejo de los pacientes con sospecha de ACG en el hospital universitario CEMIC en Buenos Aires, Argentina, y correlacionar el comportamiento de la enfermedad con el resultado de la biopsia de arteria temporal (BAT). Métodos Estudio retrospectivo que analizó pacientes consecutivos a los cuales se les realizó BAT en un período de 11 años (2005-2016). La información recolectada se obtuvo a partir de las historias clínicas y de los informes de anatomía patológica. Se realizó estadística descriptiva. Para las variables cuantitativas se estimaron medias y sus respectivos desvíos estándar o medianas y percentil 25-75, y para las variables cualitativas, la cantidad y el porcentaje. Para comparar las características de los grupos se realizaron análisis bivariados mediante tablas de contingencia y modelos de regresión logística de ser necesario. Resultados Sesenta y tres pacientes fueron incluidos, 68% mujeres, con una edad media de 72 años (DS 8,4). Diecisiete biopsias (26,9%) fueron positivas. La longitud media posfijación fue de 1,68 cm (DS 1,2). La población se dividió en 3 grupos según la BAT, los criterios ACR y los estudios por imágenes. No se identificaron factores predictores de positividad de la BAT. El grupo con ACG y BAT negativa presentó mayor porcentaje de pacientes con arteria temporal anormal al examen físico. Conclusión El porcentaje de positividad de las biopsias (26,9%) fue similar al reportado por otras series, así como la longitud de la biopsia luego de la fijación (1,68 cm). Historicamente utilizada como afrodisíaco, vesicante e abortivo. A intoxicação por este agente encontra-se amplamente descrita, tipicamente com envolvimento das mucosas gastrointestinal, com hemorragia digestiva, e genitourinária, com hematúria e lesão renal aguda. Os autores apresentam o caso de um doente do sexo masculino, com 51 anos que desenvolveu um quadro de intoxicação aguda por cantaridina, após ingestão de uma preparação (‘chá’) desta substância, com ardor na cavidade oral, diarreia, disúria, hematúria, lesão renal aguda e com um quadro neurológico atípico. A toxicidade pela cantaridina é sisté****, contudo, o envolvimento neurológico é raro. Dada ausência de um antídoto, preconiza-se a terapêutica de suporte. Este caso reforça a importância de uma anamnese detalhada, incluindo a utilização de produtos ‘naturais’ ou de ervanária (i.e. de origem não controlada), e da necessidade de alertar os doentes para a sua potencial toxicidade.

Data were analyzed using SPSS software version 20. Results The results showed that there was no significant relationship between any of the variables with being left- or right-handed. Severe pain during the defecation and emotional stress that it causes may reduce people's quality of life. Aims In this randomized clinical trial, we aimed to compare the efficiency of the topical ointment with medical treatment and surgical lateral internal sphincterotomy. Method This is a randomized clinical trial of 550 patients who were treated for chronic anal fissure. Patients were randomly divided into 4 groups according to the treatment type they received. Results In a vast majority of the patients, the primary complaint was pain (92.3%) and bleeding during defecation (62%). Both pain relief and healing of the fissure, which are the components of response to treatment, had not been observed in 56 (37.3%) patients of topical nitroglycerin ointment group until the second month. Among the recalcitrant patients in both topical nitroglycerin (56) and topical diltiazem ointment (47) groups, 27 (48.2%), and 36 (76.5%) patients underwent surgery, respectively. The best response to treatment was also obtained in lateral internal sphincterotomy group. Results The evaluation of three-dimensional (3D) images of this study showed that no significant difference was found between the percentage of MM (27.5%) and MD canals (22.5%) (P = 0.2064); however, there was a significant difference between the percentage of teeth having both extra canals (10%) and teeth having only one of these canals (P less then 0.05). Data were analyzed using SPSS software version 20. Results The results showed that there was no significant relationship between any of the variables with being left- or right-handed. Method This is a randomized clinical trial of 550 patients who were treated for chronic anal fissure. Patients were randomly divided into 4 groups according to the treatment type they received. Results In a vast majority of the patients, the primary complaint was pain (92.3%) and bleeding during defecation (62%). Both pain relief and healing of the fissure, which are the components of response to treatment, had not been observed in 56 (37.3%) patients of topical nitroglycerin ointment group until the second month. Among the recalcitrant patients in both topical nitroglycerin (56) and topical diltiazem ointment (47) groups, 27 (48.2%), and 36 (76.5%) patients underwent surgery, respectively. The best response to treatment was also obtained in lateral internal sphincterotomy group. Lung adenocarcinoma is a subtype of lung cancer with high morbidity and mortality. CD44 is instrumental in many physiological and tumor pathological processes. The expression of unique single nucleotide polymorphisms (SNPs) contributes to protein dysfunction and influences cancer susceptibility. In the current study, we investigated the relationship between CD44 polymorphisms and the susceptibility to lung adenocarcinoma with or without epidermal growth factor receptor (EGFR) gene mutations. This study included 279 patients with lung adenocarcinoma. In total, six CD44 SNPs (rs1425802, rs11821102, rs10836347, rs13347, rs187115, and rs713330) were genotyped using a real-time polymerase chain reaction. We found no significant differences in genotype distribution of CD44 polymorphisms between EGFR wild-type and EGFR mutation type in patients with lung adenocarcinoma. We observed a strong association between CD44 rs11821102 G/A polymorphism and EGFR L858R mutation (odds ratio (OR) = 3.846, 95% confidence interval (CI) = 1.018-14.538; p = 0.037) compared with the EGFR wild-type group. In the subgroup of male patients with lung adenocarcinoma harboring the EGFR wild-type, both CD44 rs713330 T/C (OR = 4.317, 95% CI = 1.029-18.115; p = 0.035) and rs10836347 C/T polymorphisms (OR = 9.391, 95% CI = 1.061-83.136; p = 0.019) exhibited significant associations with tumor size and invasion. Three different Cd stress groups (C1 group 1.0 mg kg-1, C2 group 2.5 mg kg-1 and C3 group 5.0 mg kg-1) were set in the pot experiment, and the target concentration of TiO2 NPs in each group were 0 mg kg-1 (T0), 50 mg kg-1 (T1), 100 mg kg-1 (T2) and 500 mg kg-1 (T3). Plant height and biomass decreased with the increasing of Cd content in paddy soil. TiO2 NPs could lower the phytotoxicity of Cd in terms of the changes in the morphological and biochemical characteristics, especially in the tillering and booting stage. In the tillering stage, TiO2 NPs addition caused a significant increase in plant height, biomass and the total chlorophyll content in the leaves of Oryza saliva L. In the booting stage, TiO2 NPs addition caused a 15% to 32% and 24% to 48% reduction of malondialdehyde (MDA) content for the C2 and C3 group, respectively, compared to that of the respective control treatment (T0). TiO2-NPs addition reduced the activity of peroxidase (POD) in the leaves in the booting and heading stage, and the activity of catalase (CAT) in the tillering stage. Whilst aphid interactions with susceptible plants and partially resistant genotypes have been well characterized, the interactions between aphids and non-host species are not well understood. Unravelling these non-host interactions can identify the mechanisms which contribute to plant resistance. Using contrasting aphid-host plant systems, including the broad host range pest Myzus persicae (host Arabidopsis; poor-host barley) and the cereal pest Rhopalosiphum padi (host barley; non-host Arabidopsis), we conducted a range of physiological experiments and compared aphid settling and probing behaviour on a host plant vs either a non-host or poor-host. In choice experiments, we observed that around 10% of aphids selected a non-host or poor-host plant species after 24 h. Using the Electrical Penetration Graph technique, we showed that feeding and probing behaviours differ during non-host and poor-host interactions when compared with a host interaction. In the Arabidopsis non-host interaction with the cereal pest R. padi aphids were unable to reach and feed on the phloem, with resistance likely residing in the mesophyll cell layer. In the barley poor-host interaction with M. persicae, resistance is likely phloem-based as phloem ingestion was reduced compared with the host interaction. Overall, our data suggest that plant resistance to aphids in non-host and poor-host interactions with these aphid species likely resides in different plant cell layers. Future work will take into account specific cell layers where resistances are based to dissect the underlying mechanisms and gain a better understanding of how we may improve crop resistance to aphids.Objective This review collates the published reports that focus on microbial and viral illnesses that can be transmitted by breast milk, donor milk and powdered infant formula (PIF). In this context, we attempt to define a risk framework encompassing those hazards, exposure scenarios, vulnerability and protective factors. Design A literature search was performed for reported cases of morbidity and mortality associated with different infant feeding modes. Setting Exclusive breast-feeding is the recommended for infant feeding under 6 months, or failing that, provision of donated human milk. However, the use of PIF remains high despite its intrinsic and extrinsic risk of microbial contamination, as well as the potential for adverse physiological effects, including infant gut dysbiosis. Results Viable pathogen transmission via breast-feeding or donor milk (pasteurised and unpasteurised) is rare. However, transmission of HIV and human T-cell lymphotropic virus-1 is a concern for breast-feeding mothers, particularly for mothers undertaking a mixed feeding mode (PIF and breast-feeding). In PIF, intrinsic and extrinsic microbial contamination, such as Cronobacter and Salmonella, remain significant identifiable causes of infant morbidity and mortality. Conclusions Disease transmission through breast-feeding or donor human milk is rare, most likely owing to its complex intrinsically protective composition of human milk and protection of the infant gut lining. Contamination of PIF and the morbidity associated with this is likely underappreciated in terms of community risk. The objective of this study was to assess the test-retest reliability of a baseline concussion test battery in cadets at U.S. Service Academies. Methods All cadets participating in the Concussion Assessment, Research, and Education (CARE) Consortium investigation completed a standard baseline battery that included memory, balance, symptom, and neurocognitive assessments. Annual baseline testing was completed during the first 3 years of the study. A two-way mixed-model analysis of variance (intraclass correlation coefficent (ICC)3,1) and Kappa statistics were used to assess the stability of the metrics at 1-year and 2-year time intervals. Whilst aphid interactions with susceptible plants and partially resistant genotypes have been well characterized, the interactions between aphids and non-host species are not well understood. Unravelling these non-host interactions can identify the mechanisms which contribute to plant resistance. Using contrasting aphid-host plant systems, including the broad host range pest Myzus persicae (host Arabidopsis; poor-host barley) and the cereal pest Rhopalosiphum padi (host barley; non-host Arabidopsis), we conducted a range of physiological experiments and compared aphid settling and probing behaviour on a host plant vs either a non-host or poor-host. In choice experiments, we observed that around 10% of aphids selected a non-host or poor-host plant species after 24 h. Using the Electrical Penetration Graph technique, we showed that feeding and probing behaviours differ during non-host and poor-host interactions when compared with a host interaction. In the Arabidopsis non-host interaction with the cereal pest R. padi aphids were unable to reach and feed on the phloem, with resistance likely residing in the mesophyll cell layer. In the barley poor-host interaction with M. persicae, resistance is likely phloem-based as phloem ingestion was reduced compared with the host interaction. Overall, our data suggest that plant resistance to aphids in non-host and poor-host interactions with these aphid species likely resides in different plant cell layers. Future work will take into account specific cell layers where resistances are based to dissect the underlying mechanisms and gain a better understanding of how we may improve crop resistance to aphids.Objective This review collates the published reports that focus on microbial and viral illnesses that can be transmitted by breast milk, donor milk and powdered infant formula (PIF). In this context, we attempt to define a risk framework encompassing those hazards, exposure scenarios, vulnerability and protective factors. Design A literature search was performed for reported cases of morbidity and mortality associated with different infant feeding modes. Setting Exclusive breast-feeding is the recommended for infant feeding under 6 months, or failing that, provision of donated human milk. However, the use of PIF remains high despite its intrinsic and extrinsic risk of microbial contamination, as well as the potential for adverse physiological effects, including infant gut dysbiosis. Results Viable pathogen transmission via breast-feeding or donor milk (pasteurised and unpasteurised) is rare. However, transmission of HIV and human T-cell lymphotropic virus-1 is a concern for breast-feeding mothers, particularly for mothers undertaking a mixed feeding mode (PIF and breast-feeding). In PIF, intrinsic and extrinsic microbial contamination, such as Cronobacter and Salmonella, remain significant identifiable causes of infant morbidity and mortality. Methods All cadets participating in the Concussion Assessment, Research, and Education (CARE) Consortium investigation completed a standard baseline battery that included memory, balance, symptom, and neurocognitive assessments. Annual baseline testing was completed during the first 3 years of the study. A two-way mixed-model analysis of variance (intraclass correlation coefficent (ICC)3,1) and Kappa statistics were used to assess the stability of the metrics at 1-year and 2-year time intervals. Results Of the 90 heroin-dependent patients, 74 experienced headache attacks within 2 hours of heroin use, and the headaches subsided within 72 hours of discontinuation of heroin use. Most heroin-induced headaches were similar to migraines and manifested as pulsating pain in 54 patients (51/74, 68.9%); bilateral pain was reported by 46 patients (46/74, 62.2%). Approximately half of the patients with heroin-induced headaches also reported accompanying symptoms of nausea, vomiting, and light and sound sensitivity. This study proposes a sensitive and baseline-free method to evaluate the health status of a 1018 steel I-beam by measuring its material nonlinearity using a new nonlinearity parameter defined for Rayleigh waves. This parameter yields a true value of material nonlinearity using the Rayleigh wave harmonics obtained from the experiments carried out at the intact and impacted states of the I-beam. Accordingly, the evaluated nonlinearities are inherent and damaged induced respectively. The results show that, for an intact state, the nonlinearity obtained using the new parameter and the experimental results for different propagation distances, consist of several peaks and the first peak reaches the true material nonlinearity. Whereas, in case of damaged state, the nonlinearity parameter at the impacted location shows a sudden increase and reaches a value higher than that of the nonlinearity evaluated at the same location for intact state. Thus, the health status can be easily tracked by comparing the nonlinearity obtained from the current state of the I-beam at its first peak with that of a physics based nonlinearity parameter evaluated at the intact state using the higher order elastic coefficients of the material. Therefore, this method is termed as baseline-free. Lastly, a novel concept of evaluating the population of dislocations formed in the material as a result of impact loading, using the new nonlinearity parameter is introduced and an equation for its estimation is given. The trend of the results given by this new equation are in accordance with those reported in the literature. In contrast, deviation between the linear parameter such as the wave velocity at the intact and impacted state remains marginal. Thus, by using the new nonlinearity parameter, it has been proven that the inspected steel specimen can be easily differentiated whether it is at the intact or damaged state. OBJECTIVE AND DESIGN We performed an experimental, analytical and prospective study to evaluate the systemic activation of inflammasome in atherosclerosis' patients, in order to shed light into responsible mechanisms for plaque formation. SUBJECTS We included sixty individuals distributed into 3 groups 2 groups based on the report from the angiography (severe lesions - SL and primary lesions - PL) and 1 group enclosing healthy individuals (HC). METHODS The expression assays of inflammasome genes NLRP1, NLRC4, CASP-1 and IL-1β were performed using Real Time qPCR, with specific Taqman Assays. IL-1β serum levels were analysed by commercial kit. Were applied the Shapiro-Wilk and Student's T-test as statistical tests. IL-1β gene expression was identified in our study as an important systemic detectable marker of plaque severity. BACKGROUND Marginal ulcer is a well-known complication after pancreatoduodenectomy. In light of increasing long-term survival after pancreatoduodenectomy, the identification of risk factors and preventive strategies are of utmost importance. We assessed the incidence, clinical impact, and potential risk factors of marginal ulcer after pancreatoduodenectomy. METHODS A prospectively maintained database of patients undergoing pancreatoduodenectomy was analyzed retrospectively. Univariate and bivariate competing-risk Cox regression analyses were performed to identify risk factors for marginal ulcer. RESULTS Two hundred and fifty-five consecutive patients underwent pancreatoduodenectomy. The median follow-up was 35.7 months. Marginal ulcer was diagnosed in 19 patients (7.5%), and the median time from pancreatoduodenectomy to marginal ulcer diagnosis was 450 days. Thirteen of these 19 patients presented with abdominal pain, melena, or anemia. In all these 13 patients, marginal ulcer resolved with proton pump inhibitor therapy and endoscopic surveillance. Six patients with marginal ulcer presented with an acute abdomen and underwent emergency laparotomy for marginal ulcer perforation and peritonitis. There was no marginal ulcer-related mortality. Univariate and bivariate competing-risk analyses showed an increased risk for marginal ulcer with discontinuation of proton pump inhibitor therapy, smoking, alcohol intake, and the use of non-steroidal anti-inflammatory drugs. Discontinuation of proton pump inhibitor therapy was an independent risk factor for marginal ulcer development. CONCLUSION Marginal ulcer is a relevant long-term complication after pancreatoduodenectomy that occurs more frequently after proton pump inhibitor therapy is discontinued. Based on our data, permanent use of proton pump inhibitor after pancreatoduodenectomy is strongly recommended especially for those patients who smoke, consume alcohol, or use non-steroidal anti-inflammatory drugs. We describe a case of eruptive foreign body granulomas following injection of Ellansé® that were successfully treated using methotrexate. CASE REPORT A 47-year-old woman received injections of Ellansé® for the wrinkled aspect of her cheeks. She had previously undergone injections of hyaluronic acid on the nasolabial folds. Nine months after the Ellansé® injections, the patient consulted for the recent appearance of multiple nodules on her face. Histological analysis of one of these nodules confirmed the presence of foreign-body granulomas developed in contact with spherical gaps of a size substantially identical to the Ellansé® vacuoles. Methotrexate 10mg per week for 3 months followed by 20mg per week for 9 months resulted in complete regression of the nodules. DISCUSSION Ellansé® is composed of two biocompatible and bioabsorbable polymers carboxymethylcellulose, responsible for immediate volume creation, and polycaprolactone, which promotes collagen synthesis.

OBJECTIVE AND DESIGN We performed an experimental, analytical and prospective study to evaluate the systemic activation of inflammasome in atherosclerosis' patients, in order to shed light into responsible mechanisms for plaque formation. SUBJECTS We included sixty individuals distributed into 3 groups 2 groups based on the report from the angiography (severe lesions - SL and primary lesions - PL) and 1 group enclosing healthy individuals (HC). METHODS The expression assays of inflammasome genes NLRP1, NLRC4, CASP-1 and IL-1β were performed using Real Time qPCR, with specific Taqman Assays. IL-1β serum levels were analysed by commercial kit. Were applied the Shapiro-Wilk and Student's T-test as statistical tests. According to stenosis severity, patients with primary lesions showed upregulation of inflammasome genes NLRP1 (+2.87 FC, p = 0.0008), NLRC4 (+6.34 FC, p = 4.134 × 10-07) and IL-1β (+3.39 FC, p = 0.0012) with respect to the HC group. No statistical difference was found in IL-1β serum levels according the assessed groups. CONCLUSIONS Inflammasome activation in atherosclerosis's patients can be systemic altered and may be triggered by NLRP1 and NLRC4 receptors. IL-1β gene expression was identified in our study as an important systemic detectable marker of plaque severity. BACKGROUND Marginal ulcer is a well-known complication after pancreatoduodenectomy. In light of increasing long-term survival after pancreatoduodenectomy, the identification of risk factors and preventive strategies are of utmost importance. We assessed the incidence, clinical impact, and potential risk factors of marginal ulcer after pancreatoduodenectomy. METHODS A prospectively maintained database of patients undergoing pancreatoduodenectomy was analyzed retrospectively. Univariate and bivariate competing-risk Cox regression analyses were performed to identify risk factors for marginal ulcer. RESULTS Two hundred and fifty-five consecutive patients underwent pancreatoduodenectomy. The median follow-up was 35.7 months. Marginal ulcer was diagnosed in 19 patients (7.5%), and the median time from pancreatoduodenectomy to marginal ulcer diagnosis was 450 days. Thirteen of these 19 patients presented with abdominal pain, melena, or anemia. In all these 13 patients, marginal ulcer resolved with proton pump inhibitor therapy and endoscopic surveillance. Six patients with marginal ulcer presented with an acute abdomen and underwent emergency laparotomy for marginal ulcer perforation and peritonitis. There was no marginal ulcer-related mortality. Univariate and bivariate competing-risk analyses showed an increased risk for marginal ulcer with discontinuation of proton pump inhibitor therapy, smoking, alcohol intake, and the use of non-steroidal anti-inflammatory drugs. Discontinuation of proton pump inhibitor therapy was an independent risk factor for marginal ulcer development. CONCLUSION Marginal ulcer is a relevant long-term complication after pancreatoduodenectomy that occurs more frequently after proton pump inhibitor therapy is discontinued. Based on our data, permanent use of proton pump inhibitor after pancreatoduodenectomy is strongly recommended especially for those patients who smoke, consume alcohol, or use non-steroidal anti-inflammatory drugs. We describe a case of eruptive foreign body granulomas following injection of Ellansé® that were successfully treated using methotrexate. CASE REPORT A 47-year-old woman received injections of Ellansé® for the wrinkled aspect of her cheeks. She had previously undergone injections of hyaluronic acid on the nasolabial folds. Nine months after the Ellansé® injections, the patient consulted for the recent appearance of multiple nodules on her face. Histological analysis of one of these nodules confirmed the presence of foreign-body granulomas developed in contact with spherical gaps of a size substantially identical to the Ellansé® vacuoles. Methotrexate 10mg per week for 3 months followed by 20mg per week for 9 months resulted in complete regression of the nodules. The purpose of this study was to assess the effect of downsampling the acoustic signal on the accuracy of linear-predictive (LPC) formant estimation. Based on speech produced by men, women, and children, the first four formant frequencies were estimated at sampling rates of 48, 16, and 10 kHz using different anti-alias filtering. With proper selection of number of LPC coefficients, anti-alias filter and between-frame averaging, results suggest that accuracy is not improved by rates substantially below 48 kHz. The weighting is based on an integral of Bessel-spherical harmonic modes over the entire control region. A detailed analysis of the weighting function is provided to show that the proposed method controls all the reproduction modes present on the 2D plane to minimize the reproduction error. The method is validated in both simulation-based and hardware-based experiments. Clean speech was mixed with 15 and 10 dB SNR LCN and presented to five CI users. First, speech intelligibility was analyzed as a function of increasing LE and decreasing SNR. Results indicate significant improvements (p less then 0.05) with Lombard speech intelligibility in noise conditions for 80 and 90 dB SPL. Next, an offline perturbation strategy was formulated to modify/perturb neutral speech so as to mimic LE through amplification of highly intelligible segments, uniform time stretching, and spectral mismatch filtering. This process effectively introduces aspects of LE into the neutral speech, with the hypothesis that this would benefit intelligibility for CI users. This was a meta-analysis that examined the prevalence of poor sleep quality and its correlates among perinatal and postnatal women. Methods A systematic search of both international and Chinese databases (PubMed, EMBASE, PsycINFO, Web of Science, CNKI, and Wangfang) was performed. Studies with data on sleep quality measured by the Pittsburgh Sleep Quality Index (PSQI) were included. Results Forty-two studies were included for analyses. The prevalence of poor sleep quality was 54.2% (95% CI 47.9-60.5%) in perinatal and postnatal women, with 44.5% (95% CI 37.6-51.6%) in perinatal women and 67.2% (95% CI 57.6-75.5%) in postnatal women. The pooled total PSQI score was 7.54 ± 0.40 (95% CI 6.75-8.33), while the average PSQI component scores varied from 0.13 ± 0.04 for use of sleeping medication to 1.51 ± 0.17 for habitual sleep efficiency. Maternal age, study site, survey year, comorbidity, PSQI cut-off value, and quality assessment score had significant moderating effects on the prevalence of poor sleep quality. Conclusion Given the negative impact of poor sleep quality on health outcomes and well-being, regular screening for poor sleep quality and effective interventions should be conducted for this population. Previous studies of indirect/implicit processing of threatening faces have shown that support vector machine (SVM) pattern recognition significantly differentiates individuals with SAD from healthy participants, though evidence for the role of the fear circuit in classification has been inconsistent. We extend this literature by using SVM during direct face processing. Individuals with SAD (n=47) and healthy controls (n=46) completed a validated emotional face matching task during functional MRI, which included a matching shapes control condition. SVM was based on brain response to threat (vs. happy) faces, threat faces (vs. shapes), and threat/happy faces (vs. shapes) in 90 regions encompassing frontal, limbic, parietal, temporal, and occipital systems. Recursive feature elimination (RFE) was used for feature selection and to rank the contribution of regions in predicting SAD diagnosis. SVM results for threat (vs. happy) faces revealed satisfactory accuracy (e.g., area under the curve=0.72); results with shapes as "baseline" yielded less optimal classification. The aim of this study was to analyze the predictors of adherence to a self-help internet-delivered intervention for adjustment disorder. Methods This was a secondary report of data, including unpublished data, from a randomized controlled trial of an internet-delivered self-help intervention for adjustment disorder. The study included 1,077 participants who had completed online baseline assessments. All participants had experienced significant life stressors over the last 2 years and had high levels of adjustment disorder symptoms. We analyzed the role of sociodemographic variables, pre-treatment adjustment disorder symptoms, outcome expectations, and perceived barriers to mental health services on the use of the intervention. Results We found that usage of internet-delivered self-help intervention and higher adherence was associated with female gender, greater age, higher pre-intervention outcome expectations, exposure to other forms of psychological therapy in addition to the internet-intervention at the time of the study, and reported perceived barriers to mental health services by the study participants. Conclusions The findings of the study indicated the importance of non-specific therapeutic factors on adherence during internet-delivered intervention. Perceived barriers to mental health services were associated with higher adherence to self-help intervention, which indicated that communities with restricted access to mental health services could benefit from low-intensity internet-delivered interventions. The current study analyzed the link between different coping strategies and suicidality in university students in China. Methods A cross-sectional study of 2,074 undergraduate students from China used a stratified-clustered-random sampling method (response rate 94.4%). The Suicidal Behaviors Questionnaire-Revised Scale was used to identify suicidal risks, while the Brief COPE scale was used to measure different coping strategies. Univariate and multivariate logistic regression analyses were utilized to examine coping strategies and suicidality. Results A negative association of some coping skills (active coping and positive reframing) with suicidality and a positive association of some other coping skills (self-distraction, substance abuse, behavioral disengagement, venting, and self-blame) with suicidality were observed after adjusting for sociodemographic and mental health variables.

The Suicidal Behaviors Questionnaire-Revised Scale was used to identify suicidal risks, while the Brief COPE scale was used to measure different coping strategies. Univariate and multivariate logistic regression analyses were utilized to examine coping strategies and suicidality.
