    A year-long randomized, prospective cohort study was conducted in 2016-2017 with pairs of resident mentors from seven different training programs and medical student mentees, randomized to telementorship or in-person mentorship.     A major component of the ACGME's Next Accreditation System (NAS) is the annual review of key performance indicators by each review committee (RC) for all programs under its oversight. The RC may request a site visit that is data-prompted for either a full review of all common and specialty-specific program requirements or a focused review of specific concerns for programs identified as underperforming.

    The aims of this study were to (1) identify the reasons that RCs requested data-prompted site visits; (2) describe the findings by accreditation field representatives as reflected in their site visit reports; and (3) summarize the accreditation decisions of RCs that followed the data-prompted site visits (DPSVs).

    RC letters to programs informing them of a DPSV, site visit reports, and RC letters with accreditation decisions were reviewed for all programs having DPSVs from 2015 to 2020.

    DPSVs were performed in 312 programs, including 59 hospital-based, 122 medical-based, and 131 surgery-based programs; 214 programs had a single DPSV, and 98 programs had repeat DPSV. The most frequent reason that RCs requested a DPSV was noncompliance on the annual ACGME Resident/Fellow Survey. Notification of a DPSV prompted a change in program director in 7% of programs in the single DPSVs group and 57% of programs in the repeat DPSVs group. Surgery-based programs in the single and repeat DPSVs groups were more likely to receive an unfavorable accreditation status. The majority of programs in the single DPSVs group (78%) and repeat DPSVs group (70%) had a status of continued accreditation as of March 2020.

    We provide evidence for a curriculum and assessment designed to standardize the training process and ensure ICD competency in surgical interns.

    In March 2019, PowerPoint educational materials were emailed to one academic institution's new surgical interns, who in June participated in an onsite 1-hour role-play "hot seat" group activity (GA) with an untrained
    , and in October completed a single trained simulated patient (real-time raters) verification of proficiency (VOP) assessment. Curriculum evaluation was measured through intern pre-/post-confidence (5-point scale), and the VOP's Cronbach's alpha and test-retest were examined. Data were analyzed with descriptive statistics, paired
    tests, and 2-way random effects models.

    Of 44 new interns, 40 (91%) participated in the remote teaching and live GA and were assessed by the VOP. Pre-/post-GA confidence increased a mean difference of 1.3 (SD = 0.63,
    < .001). The VOP's Cronbach's alpha was 0.88 and test-retest was 0.84 (95% CI 0.67-0.93,
    < .001), with a 95% pass rate. The 2 first-time fail students required remediation. Time commitment included 1 hour maximum for individual training and implementation and 30 minutes for assessment. The use of volunteers and donated space mitigated additional costs.

One of the goals of AMA-ACE was to support HSS curricular innovation to improve residency preparation.

  • Toscana virus (TOSV) is an emerging sandfly-borne virus within the Phlebovirus genus. Although most infections caused by this virus present as asymptomatic or with minimal symptomatology, TOSV may emerge as a febrile disease or sporadic cases of neurological disease such as meningitis or meningoencephalitis. This pathogen is distributed throughout the Mediterranean basin, along with the spatial distribution of its recognized sandfly vector, Phlebotomus perniciosus. Portugal, after Italy, was the second country considered endemic for this virus, with the first case of acquired infection published in 1985. Although little is known about the circulation of this virus in Portugal, the laboratory diagnosis of TOSV is available at the Centre for Vectors and Infectious Diseases Research of the National Institute of Health Dr. Ricardo Jorge (CEVDI/INSA), since 2007. The aim of this study is to report the results of the diagnosis of TOSV at the CEVDI/INSA, between 2009 and 2018.

    Toscana virus (TOSV) is an emerging sandfly-borne virus within the Phlebovirus genus.     One hundred and five normal vaginal deliveries, 19 vacuum/forceps deliveries, and 17 cesarean deliveries were included. In the normal delivery group, the rate of puborectalis muscle trauma was low and showed no significant change between the 24th and 34th weeks of gestation (12.5% vs. 17.2%, p = 0.42). The rate of trauma significantly increased to 70.2% at the 5th day postpartum (p < 0.001). There was no significant difference between the rates at the 5th day postpartum and 1month postpartum (73.7%, p = 0.60). The same trend was found in the vacuum/forceps group. In the cesarean section group, no significant change was observed throughout pregnancy and postpartum periods.     Medline, Embase and The Cochrane Library were searched to identify all patient- and clinician-reported outcomes in studies assessing medical, surgical or combination treatment of adult patients with AF published from January 2008 to May 2020. Tetracycline hydrochloride and orange II were implemented as model pollutants to evaluate the efficacy of the GO/MOFs in water purification, in which 50 mg of Zr/Fe-MOFs/GO was mixed with 100 mL of 10 mg/L, 20 mg/L, 30 mg/L, or 50 mg/L tetracycline hydrochloride solution and 25 mg/L, 35 mg/L, 45 mg/L, or 60 mg/L orange II solution, respectively. The removal efficacy after 4 hours was determined to be 96.1%, 75.8%, 55.4%, and 30.1%, and 98.8%, 91.9%, 71.1%, and 66.2%, respectively. The kinetics of pollutant removal was investigated for both tetracycline hydrochloride and orange II and excellent correlation coefficients of greater than 0.99 were obtained. The high efficacy of these MOFs in pollutant removal, coupled with their inexpensive preparation indicates the feasibility of their implementation in strategies for treating waste liquid.     Although the pedicle is routinely used as a surgical fixation site, the pedicle wall bone area fraction (bone area per unit area) and its distribution at the isthmus of the pedicle remain unknown. The bone area fraction at the pedicle isthmus is an important factor contributing to the strength of pedicle screw constructs. This study investigates the lumbar pedicle wall microstructure based on micro-computed tomography.

    Six fresh-frozen cadaveric lumbar spines were analyzed. Left and right pedicles of each vertebra from L1 to L5 were resected for micro-computed tomography scanning. Data was analyzed with custom-written software to determine regional variation in pedicle wall bone area fraction. The pedicular cross-section was divided into four regions lateral, medial, cranial, and caudal. The mean bone area fraction values for each region were calculated for all lumbar spine levels.

    The lateral region showed lower bone area fraction than the medial region at all spinal levels. Bone area fraction in the medial region was the highest at all levels except for L4, and the median values were 99.8% (95.9-100%). There were significant differences between the lateral region and the caudal region at L1, L2 and L3, but none at L4 and L5.     Graphene has shown great potential for improving growth of many plants, but its effect on woody plants remains essentially unstudied.     To provide an analysis of the concept of medication errors that occur during the nursing task of medication administration.

    Medication errors occurring during medication administration have an increased risk for an adverse patient outcome. Many terms are associated with medication errors, with limited clarification for a medication administration error at the point of care delivery.

    Twenty-seven sources were chosen from database searches of PubMed, CINAHL, EMBASE, and Scopus. Key search terms used to narrow the search consist of the following terminology ("bedside" OR "point-of-care" OR "nursing care*") AND "medication administration errors" AND ("acute care").

    Rodger's evolutionary method of concept analysis was used to clarify the concept of medication administration error.

    The concept analysis identified three attributes of a medication administration error nurse-provided care, administration, and point of care. Antecedents identified in the analysis are organizational factors and nurse factors. Consequences identified in the analysis are patient, nurse and organizational themes.

    The continued occurrence of medication administration errors warrants the need for an investigation into existing safety measures. Using this concept analysis may help guide the study of the healthcare environment and determine empirical measures to assess the impact that a medication administration error has.

I respond to this by developing an interactionist perspective on embodied elite distinction not as a function of social position, but rather as means of communication used more or less actively to manage others' impressions of one's social position. Drawing on ethnography and interviews, I explore how students at two Swedish upper secondary schools with different elite profiles-an economic elite school and a meritocratic elite school-negotiate what counts as successful performances of their elite position in face-to-face interactions during conferences for "young leaders". The findings suggest that in order for performances to be honored as "confident ease," which is attributed with prestige and moral legitimacy in the peer group, the interacting students had to share the impression that the interaction was a "merit situation". In interactions defined as "privilege situations," similar behavior was labelled as arrogant superiority and seen as illegitimate performances of elite position. While "confident ease" was the hallmark of the male students at the economic elite school, the findings suggests that "confident awareness" may be understood as a contradictory mode of elite distinction characterizing the female students from the meritocratic elite school. They display confidence while simultaneously exploiting the unease they felt in the conference to craft a morally legitimate performance of their elite position. While the findings confirm that students' classed, gendered and moral dispositions are central to navigate elite distinction, they also demonstrate that meanings of such embodied distinctions emerge in relation to how situations are defined and are dependent on the interacting students' combined ability and interest to maintain working consensus on the definition of the situation.Miso is a well-known traditional Japanese fermented food, with a characteristic savory flavor and aroma, known predominately as the seasoning in miso soup. Miso production involves a two-stage fermentation, where first a mold, such as Aspergillus oryzae, is inoculated onto a substrate to make koji. A subsequent fermentation, this time by bacteria and yeast, occurs when the koji is added to a salt and soybean mash, with the miso left to ferment for up to 2 years. The microbial community of miso is considered essential to the development of the unique taste, texture, and nutritional profile of miso. Despite the importance of microorganisms in the production of miso, very little research has been undertaken to characterize and describe the microbial process. In this review, we provide an overview of the two-stage fermentation process, describe what is currently known about the microbial communities involved and consider any potential health benefits associated with the consumption of miso, along with food safety concerns. As the popularity of miso continues to expand globally and is produced under new environmental conditions, understanding the microbiological processes involved will assist to ensure that global production of miso is safe as well as delicious.

  • New solutions for the dynamic assessment of the moving structures of the temporomandibular joint (TMJ), such as the intra-articular disc and the head of the mandible, are required for magnetic resonance imaging (MRI). Therefore, we propose a simple, graded bite block for dynamic MRI examination of the TMJ.

    The bite block consists of three main functional elements a groove for the biting edges of the upper incisors, steps with bite surfaces for the biting edges of the lower incisors and a handle for easy placement and control of the device. The block is easily manufactured using commercially available 3D printers. The closed-mouth part of the study was performed without the bite block. The patient was instructed to place the central incisors' biting edges in the upper groove. The lower incisors' biting edges were placed consecutively on the first step, second step, and so on, in increasing order, until maximal opening was reached.

    Ninety-five MR examinations of TMJs were performed. The graded opening bite block was successfully applied in all but two patients.

    In conclusion, the simple and easy-to-use graded bite block may be a useful addition to the armamentarium of a radiologist performing MRI scans of the TMJ.

Thus, this study aimed to examine the associations of dietary B vitamins intakes with the risk of depression in American adults. Methods The data we used in this study were from the National Health and Nutrition Examination Survey (NHANES) 2007-2014. We used the Logistic regression models to analyze the associations of the dietary intakes of B vitamins with the risk of depression. Results 17,732 individuals (8,623 males and 9,109 females) were enrolled in the study and they were all 18 or older. Compared to the lowest quartile of dietary intake, the ORs (95%CIs) of the highest quartile were 0.64 (0.50-0.82), 0.78 (0.62-0.97), 0.60 (0.47-0.78), 0.65 (0.50-0.84), and 0.71 (0.54-0.95) for vitamin B1, vitamin B2, niacin, vitamin B6, and vitamin B12, respectively. Compared to the people whose dietary intakes below the RDA in the model 2, those with intake meeting the RDA of vitamin B1 (OR 0.68; 95%CI 0.56-0.84), niacin (OR 0.65; 95%CI 0.51-0.81), B6 (OR 0.65; 95%CI 0.52-0.81), or B12 (OR 0.65; 95%CI 0.48-0.88) had a lower risk of depression, severally. We also found a nonlinear negative association between dietary vitamin B1, vitamin B2, niacin, vitamin B6, and vitamin B12 intakes and the risk of depression in the dose-response analyses, severally. Conclusions Our results suggested that dietary vitamin B1, vitamin B2, niacin, vitamin B6, and vitamin B12 intakes may be inversely associated with the risk of depression.
    The Neck Imaging Reporting and Data System (NI-RADS) is an increasingly utilized risk stratification tool for imaging surveillance after treatment for head and neck cancer. This study aims to measure the impact of supervision by subspecialized radiologists on diagnostic accuracy of NI-RADS when initial reading is performed by residents.

    150 CT and MRI datasets were initially read by two trained residents, and then supervised by two subspecialized radiologists. Recurrence rates by NI-RADS category were calculated, and receiver operating characteristic (ROC) curves were plotted. After dichotomization of the NI-RADS system (category 1

    As it is well known, the diagnosis of temporomandibular joint (TMJ) involvement in patients affected by Juvenile Idiopathic Arthritis (JIA) is important to avoid the impairment of mandibular growth. In this context, Magnetic Resonance Imaging (MRI) is the gold-standard for detection of TMJ involvement, however it is expensive and requires patients' collaboration. The aim of this study was to evaluate if ultrasound may be used as an alternative tool to investigate the acute signs of TMJ involvement in JIA patients.

    Lateral periarticular space (LPAS) and joint effusion were evaluated by ultrasound in a study Group A of 8 JIA children (11.6±3.5 years old) with 14 TMJs involved, as confirmed by MRI, and in a control Group B of 7 healthy children (9.3±1.2 years old) without temporomandibular disorders (TMD). The LPAS width values were compared between the two groups using the Mann-Whitney test. The ultrasound images of the JIA group were then matched with the corresponding MR images; the Spearman Rank Correlation test and the Bland-Altman test were used to evaluate the differences.

    The LPAS values in Group A were statistically significantly higher than those in Group B (
    < 0.001). There was no overlap of the LPAS values confidence intervals (CIs) between the two groups. Compared to the lowest quartile of dietary intake, the ORs (95%CIs) of the highest quartile were 0.64 (0.50-0.82), 0.78 (0.62-0.97), 0.60 (0.47-0.78), 0.65 (0.50-0.84), and 0.71 (0.54-0.95) for vitamin B1, vitamin B2, niacin, vitamin B6, and vitamin B12, respectively. Compared to the people whose dietary intakes below the RDA in the model 2, those with intake meeting the RDA of vitamin B1 (OR 0.68; 95%CI 0.56-0.84), niacin (OR 0.65; 95%CI 0.51-0.81), B6 (OR 0.65; 95%CI 0.52-0.81), or B12 (OR 0.65; 95%CI 0.48-0.88) had a lower risk of depression, severally. We also found a nonlinear negative association between dietary vitamin B1, vitamin B2, niacin, vitamin B6, and vitamin B12 intakes and the risk of depression in the dose-response analyses, severally. Conclusions Our results suggested that dietary vitamin B1, vitamin B2, niacin, vitamin B6, and vitamin B12 intakes may be inversely associated with the risk of depression. This study aims to measure the impact of supervision by subspecialized radiologists on diagnostic accuracy of NI-RADS when initial reading is performed by residents. 150 CT and MRI datasets were initially read by two trained residents, and then supervised by two subspecialized radiologists. Recurrence rates by NI-RADS category were calculated, and receiver operating characteristic (ROC) curves were plotted. After dichotomization of the NI-RADS system (category 1 categories 2 + 3+4 and categories 1 + 2 3 + 4), sensitivity, specificity, positive and negative predictive value were calculated. 26% of the reports were modified by the supervising radiologists. Area under the curve of ROC plots values of the supervision session were higher than those of the initial reading session for both the primary site (0.89 0.86) and the neck (0.94 0.91), but the difference was not statistically significant. As it is well known, the diagnosis of temporomandibular joint (TMJ) involvement in patients affected by Juvenile Idiopathic Arthritis (JIA) is important to avoid the impairment of mandibular growth. In this context, Magnetic Resonance Imaging (MRI) is the gold-standard for detection of TMJ involvement, however it is expensive and requires patients' collaboration. The aim of this study was to evaluate if ultrasound may be used as an alternative tool to investigate the acute signs of TMJ involvement in JIA patients. Lateral periarticular space (LPAS) and joint effusion were evaluated by ultrasound in a study Group A of 8 JIA children (11.6±3.5 years old) with 14 TMJs involved, as confirmed by MRI, and in a control Group B of 7 healthy children (9.3±1.2 years old) without temporomandibular disorders (TMD). The LPAS width values were compared between the two groups using the Mann-Whitney test. The ultrasound images of the JIA group were then matched with the corresponding MR images; the Spearman Rank Correlation test and the Bland-Altman test were used to evaluate the differences. The LPAS values in Group A were statistically significantly higher than those in Group B ( < 0.001). There was no overlap of the LPAS values confidence intervals (CIs) between the two groups.   • The hypothesis that the most common female endocrine disease, the polycystic ovarian syndrome (PCOS), has a male equivalent, has recently become more widely accepted. The male form of PCOS is marked by alterations in the secretion of gonadotropins, increased insulin resistance, and changes of the levels of several steroid hormones, with clinical manifestations including premature androgenic alopecia (AGA). Because these symptoms are not always found in men with genetic predispositions, knowledge of the male equivalent of PCOS needs to be supplemented by measurements of adrenal 11-oxygenated C19 steroids, particularly 11-keto-, and 11β-hydroxy-derivatives of testosterone and dihydrotestosterone, by focusing on the newly-realized role of skin as an endocrine organ, and by confirming any age-related factors in glucose metabolism disorders in such predisposed men.Extracorporeal life support has been increasingly utilized in different clinical settings to manage either critical respiratory or heart failure. Complex airway surgery with significant or even total perioperative airway obstruction represents an indication for this technique to prevent/overcome a critical period of severe hypoxaemia, hypoventilation, and/or apnea. This review summarizes the current published scientific evidence on the utility of extracorporeal respiratory support in airway obstruction associated with hypoxaemia, describes the available methods, their clinical indications, and possible limitations. Extracorporeal membrane oxygenation using veno-arterial or veno-venous mode is most commonly employed in such scenarios caused by endoluminal, external, or combined obstruction of the trachea and main bronchi.Determining appropriate care for patients who cannot speak for themselves is one of the most challenging issues in contemporary healthcare and medical decision-making. While there has been **** discussion relating to patients who left some sort of instructions, such as an advance directive, or have someone to speak on their behalf, less has been written on caring for patients who have nobody at all available to speak for them. It is thus crucial to develop clear and rigorous guidelines to properly care for these patients. The Jewish tradition offers an important perspective on caring for unrepresented patients and determining approaches to guide care providers. This article develops an understanding of fundamental Jewish principles that can provide clear guidance in navigating this challenge. It applies those values to a specific set of suggested behaviors, one of which adds a novel ritualized component to what has been recommended by bioethicists in the past.
    Our study aimed to determine the relationship between serum periostin levels, and the neutrophil-lymphocyte ratio (NLR) with ischemic stroke subtypes, clinical stroke scales, and acute prognosis in patients with acute ischemic stroke.

    Forty-two ischemic stroke patients and 39 age- and sex-matched healthy volunteers were included in our study. Demographic characteristics including age and gender were recorded. Blood serum periostin and NLR values were evaluated in the first 24 hours after admission. Serum periostin levels were compared with healthy controls of similar age and sex. Lesion localization was determined by cranial CT or diffusion MRI of the patients. Stroke scales were recorded on days 1 and 7 of hospitalization in the study group.

    The mean serum periostin levels were higher than in the control group, but no statistically significant difference was found. There was no correlation between serum periostin levels and prognosis of stroke. First admission NLRs were statistically higher than in the control group. The first admission NLRs were positively correlated with the first admission National Institute of Health Stroke Scale score and the day 7 modified Rankin score.

    Our study is the first study to evaluate both NLR and serum periostin levels in all types of acute ischemic stroke. While our study did not show that first admission serum periostin levels can be used as a biomarker in ischemic stroke, it did indicate that the first admission NLR can be used for acute prognosis of ischemic stroke.

    Since the occurrence of the SARS-COV2 pandemic, there has been an increasing interest in investigating the epidemiology of delirium. Delirium is frequent in SARS-COV2 patients and it is associated with increased mortality; however, no information is available on the association between delirium duration in SARS-COV2 patients and related outcomes.

    The aim of this study is to investigate the association between the duration of delirium symptoms and in-hospital mortality in older patients with SARS-COV2 infection.

    Retrospective cohort study of patients 65years of age and older with SARS-CoV 2 infection admitted to two acute geriatric wards and one rehabilitation ward. Delirium symptoms duration was assessed retrospectively with a chart-based validated method. In-hospital mortality was ascertained via medical records.

    This study included 146 adult scoliosis patients aged 20-50years (main curve in thoracic spine). Respiratory function was assessed as predicted forced vital capacity (%FVC) and the ratio of forced expiratory volume in 1s / FVC (%FEV
    ) preoperatively and 2years postoperatively and classified as a normal function (≥ 80%), mild impairment (≥ 65% and < 80%), and moderate impairment (< 65%).

    Preoperative %FVC and %FEV

  • Melanoma is one of the most malignant skin cancers that require comprehensive therapies, including chemotherapy. A plant-derived drug, plumbagin (PLB), exhibits an anticancer property in several cancers. We compared the cytotoxic and metabolic roles of PLB in A375 and SK-MEL-28 cells, each with different aggressiveness. In our results, they were observed to have distinctive mitochondrial respiratory functions. The primary reactive oxygen species (ROS) source of A375 can be robustly attenuated by cell membrane permeabilization. A375 cell viability and proliferation, migration, and apoptosis induction are more sensitive to PLB treatment. PLB induced metabolic alternations in SK-MEL-28 cells, which included increasing mitochondrial oxidative phosphorylation (OXPHOS), mitochondrial ATP production, and mitochondrial mass. Decreasing mitochondrial OXPHOS and total ATP production with elevated mitochondrial membrane potential (MMP) were observed in PLB-induced A375 cells. PLB also induced ROS production and increased proton leak and non-mitochondria respiration in both cells. This study reveals the relationship between metabolism and cytotoxic effects of PLB in melanoma. This study employed a photobonding process with vacuum ultraviolet (VUV) light to fabricate microfluidic devices without using any solvent and compared their performance with that of solvent-bonded systems (using cyclohexane, dichloromethane, or toluene as the solvent) to investigate the effects of residual solvent on cell cultures. Quantitative immunofluorescence assays indicated that the coating efficiencies of extracellular matrix proteins (e.g., Matrigel and collagen I) were lower in solvent-bonded COP devices than those in VUV-bonded devices. Furthermore, the cytotoxicity of the systems was evaluated using SH-SY5Y neuroblastoma cells, and increased apoptosis was observed in the solvent-processed devices. From April 2015 to December 2016, 800 Kazakh males and females with primary hypertension who met the inclusion criteria were included in this cross-sectional study. Data were collected on socio-demographics, lifestyle parameters, family history of cardiovascular disease, and hypertension. Additionally, Dietary Quality Score (DQS), anthropometric data, and blood pressure were recorded. Laboratory blood measurements included eGFR (estimated glomerular filtration rate), lipid profile parameters such as Apolipoprotein B, A1, HDL-C, LDL-C, and TG. We found a linear relationship between early renal dysfunction and LDL-C, Apolipoprotein B, and Apolipoprotein B/A1 ratio, which was in all cases negative and small (r = -0.27, -0.23 and -0.16, respectively). Apolipoprotein A1, HDL-C and TG have not revealed a linear relationship with GFR (r = -0.06, r = -0.06, and ρ = -0.045, respectively). The multicollinearity test restricted the linear model to Apolipoprotein B only. Further linear regression analysis confirmed an inverse significant linear association between eGFR and Apolipoprotein B. Age, DQS, and income appear to be positive confounding factors, significantly fitted the final model. For example, AP-1 complexes containing Fra-1, Fra-2 and B-ATF play central roles in the transcriptional control of B cell development and plasma cell differentiation, while dysregulation of AP-1 family members c-Maf, c-Jun, and JunB is associated with MM cell proliferation, survival, drug resistance, bone marrow angiogenesis, and bone disease. The present review article summarizes our up-to-date knowledge on the role of AP-1 family members in plasma cell differentiation and MM pathophysiology. Moreover, it discusses novel, rationally derived approaches to therapeutically target AP-1 TFs, including protein-protein and protein-DNA binding inhibitors, epigenetic modifiers and natural products.
